Job Summary:
The Clinical Staff Pharmacist - Inpatient Services provides pharmaceutical services to the hospital’s inpatients through optimizing the use of pharmaceuticals, and collaborating with healthcare professionals throughout the organization to provide the highest quality of patient care. The incumbent proactively intervenes to optimize each patient's therapy and documents the outcome of the intervention, provides accurate drug information, ensures that pharmaceutical products and services are provided in a timely, safe and cost-effective manner and ensures compliance with current medication management standards. The Clinical Staff Pharmacist is centralized in the Inpatient Pharmacy or in one of the Satellite Pharmacies throughout the hospital.
KEY ROLE ACCOUNTABILITIES:
- Reviews patients’ medication orders for efficacy and safety, and makes recommendations to Prescriber if changes are required.
- Provides medication information to healthcare professionals, including but not limited to Prescribers and Nurses.
- Verifies medication orders for accuracy and clinical appropriateness, checks medications prepared by Pharmacy Technicians for accuracy prior to dispensing and notifies appropriate persons if changes are required.
- Participates in committees related to medication use and provides recommendations regarding various aspects of medication use process.
CLINICL:
- Checks and verifies that medication reconciliation is completed on admission and discharge.
- Reviews patients’ medication orders for efficacy and safety, and makes recommendations if changes are required.
- Provides drug information to healthcare professionals, including but not limited to prescribers and nurses.
- Performs therapeutic drug level monitoring and makes adjustments for ensuring patient safety.
- Consults with peers and supervisors when assistance is needed in choosing the most appropriate drug therapy.
- Participates in drug use evaluation and drug protocol management.
- Assesses the status of the patient’s health problems and determines whether the prescribed medications are optimally meeting the patient’s needs and goals of care.
- Performs Drug-Drug, Drug-Lab, Drug Food and Drug-Disease Monitoring and offers alternatives when applicable.
- Ensures continuity of care between the acute and ambulatory patient care environments.
- Documents all clinical activities and interventions accurately and completely, as per policy and procedure.
COMPETENCY:
- Provides in-service education to Pharmacy and non-Pharmacy personnel.
- Mentors pharmacy students and pharmacy residents.
- Participates in the orientation and training of Pharmacy students, residents and newly hired Pharmacists and Technicians.
- Communicates effectively with healthcare professionals, patients and family members where appropriate.
- Demonstrates proficiency in the compounding of extemporaneous preparations.
- Demonstrates proficiency in aseptic technique during the preparation of sterile pharmaceutical products.
- Demonstrates working knowledge of departmental equipment and software where applicable for assigned duties.
- Has (or will obtain) certification in Basic Life Support (BLS
- Maintains professional competency and skills sufficient to meet current and future needs of the facility.
MEDICATION SAFETY:
- Monitors and reports adverse drug reactions.
- Adheres to policies and procedures relating to Medication Safety, and provides suggestions for increasing it.
- Supports and participates in the goals of the organization's quality improvement, risk management, and patient safety programs to include (but not limited to) National Patient Safety Goals, Core Measures, and other medication-related Joint Commission International (JCI) initiatives.
OPERATIONAL:
- Contributes to the achievement of goals of the department through personal effort and influence over others.
- Verifies medication orders for accuracy and clinical appropriateness, checks medications prepared by Pharmacy Technicians for accuracy prior to dispensing and notifies appropriate persons if changes are required.
- Provides accurate shift-to-shift communication regarding unsolved situations requiring future action.
- Provides professional, courteous customer service to all customer types (patients/family, Nursing, Medical Staff, others).

QUALIFICATIONS, EXPERIENCE AND SKILLS:
ESSENTIAL PREFERRED Education- BSc - Bachelor of Science in Pharmacy/BPharm or M.Sc., Master of Science in Pharmacy/MPharm
- All degrees must have been obtained from an ACPE accredited college of Pharmacy or equivalent
- Doctor of Pharmacy (Pharm.D) or Postgraduate Diploma in Clinical Pharmacy (PGDipClinPharm)
- Residency or fellowship in a pediatric specialty area
- 3+ years’ experience as a licensed/ registered Clinical Staff Pharmacist after graduation from primary degree in a recognized pediatric/women's hospital
- 0 years’ experience as a recent graduate of Doctor of Pharmacy (Pharm.D) Program at Qatar University
- Licensed in Pharmacy in country of origin
- Obtain State of Qatar Pharmacy License after on boarding
- Excellent clinical knowledge as it pertains to medication and disease management
- Demonstrated ability to work with Pharmacy Information Systems
- Proficiency with Microsoft Office suite
- Fluency in written and spoken English
